The Non-Profit Healthcare Organization (NPHO) Assistance Program will assist non-profit organizations located in Gadsden County, FL.  This program will assist NPHOs located in Gadsden County that have been impacted by COVID-19.  The Gadsden Community Health Council (GCHC) will serve as a “clearing house” for this program to identify county healthcare non-profits who would be considered for and receive the assistance funds. The GCHC is a partner with the Gadsden County Health Department (GCHD) and assists with various functions and logistics for the GCHD. The GCHC works with and donates to many organizations within Gadsden County.  NPHOs that may apply would have to provide the following services within the healthcare community: medical, food, educational, or childcare services.  The program eligibility and documentation requirements are the following:

Eligibility and Documentation Requirements:
  • Designated official of the applying NPHO must submit signed Attestation that the organization was in operation prior to March 1, 2020, is still operational, and is based in Gadsden County, FL serving the community.
  • Attest and describe the organization’s role in providing medical, food, educational and childcare services to residents in Gadsden County during the COVID-19 pandemic.
  • Submit documentation showing the organization lost donations/ revenue due to COVID-19 and describe how this loss negatively impacted the community. Or, the applying NPHO may describe additional eligible services that were provided, and expenditures incurred due to COVID-19.
  • Provide amount of loss or new costs (describing 2020 revenue or expenditures compared to 2019)
  • Submit documentation validating location in Gadsden County (utility bill, other documents)
  • Submit 501c3 determination letter or Tax Exempt Certificate.
  • All grant award totals will be reduced by the amount of any Payroll Protection Program (PPP) or other COVID 19 related funding received by the applicant.
  • Applicant must complete and submit an IRS W-9 Form prior to receipt of funding.
